    How chloroquine causes itch

Itch has many similarities to pain, and while both are unpleasant sensory experiences, their behavioral response patterns are different. Pain creates a withdrawal reflex, whereas itch leads to a scratch reflex.

The itching reactions caused by chloroquine or cowhage don't respond to anti-histamines. Certain opioids used to treat pain, such as morphine, can also cause a histamine-independent itch as a side. Lead researcher Diana Bautista collected a variety of substances that cause the itch reflex, including a plant called cowhage and the drug chloroquine, which often causes a persistent, all-over itch. The itch produced by chloroquine is so common that in some African countries 70 percent of the population has been affected by it. Investigators have used histamine along with another pruritogen, chloroquine, in an attempt to define the afferent nerve phenotype responsible for itch transduction in mice. Chloroquine is an anti-malarial drug known to evoked pruritus as one potential side-effect in humans.

Pain and itch have very different behavioral response patterns. Unmyelinated nerve fibers for itch and pain both originate in the skin; however, information for them is conveyed centrally in two distinct systems that both use the same nerve bundle and spinothalamic tract.

Chloroquine is used both as an antimalarial drug and in the treatment of rheumatological disorders, such as rheumatoid arthritis and systemic lupus erythematosus. Dec 25, 2009 The itch receptors identified by Howard Hughes Medical Institute HHMI scientists belong to a family of proteins known as Mrg receptors. “We think that chloroquine causes itching primarily by binding to an Mrg receptor on nerve fibers in skin,” said Xinzhong Dong, the senior author on the study and an HHMI Early Career Scientist at Johns Hopkins University. Prednisone and Plaquenil question" Lupus Community. Yes, if the plaquenil controls your Lupus flares, it is best to not use the prednisone. I hope you can get off of it completely, or at least take a small maintenance dose. Take it very slow, tapering off, and if you have a reaction, severe fatigue, etc, talk to your doc, about tapering down even slower.
